A 93-Year-Old Patient with Complex Cardiac and Traumatological Conditions Successfully Treated at the Central Customs Hospital

A 93-year-old patient was admitted to the hospital with a pelvic fracture and severe pain following a fall at home. During a comprehensive preoperative medical assessment prior to orthopedic surgery, several life-threatening cardiovascular conditions were identified. These included severe coronary artery stenosis, critical aortic valve stenosis (gradient of 110 mmHg), and significant renal artery stenosis.
Given the high procedural risk, a multidisciplinary medical board decided to address the cardiac conditions as a priority.
Within a single session, the Head of the Cardiology Department of the Central Customs Hospital, interventional cardiologist Ilgar Tahiroglu, and his team successfully restored coronary blood flow by implanting a stent. This was followed by a successful Transcatheter Aortic Valve Implantation (TAVI), during which a new aortic valve was placed. Subsequently, the critical renal artery stenosis was also treated using an endovascular approach.
Two days after stabilization of the patient’s cardiac condition, traumatological surgery was performed. The surgical intervention in the hip–pelvic region was successfully carried out by trauma surgeon Ramin Rzayev. All stages of treatment proceeded without complications, and the patient was discharged home pain-free within a short period.
The successful management of this complex clinical case is a clear demonstration of the high level of professionalism and effective multidisciplinary teamwork among anesthesiologists, cardiologists, interventional specialists, and surgeons.
